The Blanchard Institute is licensed by the North Carolina DHHS to provide DWI Services for individuals charged with in-state and out-of-state DWI offenses, including DWI Assessments and, when recommended or required, additional substance use education or ongoing treatment.
Our team eases the process of satisfying the DMV requirements for both the individual and family. Our DWI Assessments are offered virtually or in-person at both our Charlotte and Lake Norman facilities. We also offer additional education around substance use disorders and ongoing substance use treatment based on what is mandated by North Carolina Statutes.
Once you’ve scheduled a DWI Assessment with The Blanchard Institute, the North Carolina Department of Motor Vehicles (NC DMV) E508 process begins.

Clients will need to provide the following documents at the time of assessment:
- Proof of Blood / Breath Alcohol Concentration (BAC)
- Citation (if available)
- Current Lifetime North Carolina Motor Vehicle Record (MVR)
- $100 Nonrefundable Assessment Fee (Prepaid)
- During the assessment, The Blanchard Institute will obtain and/or provide the following information:
- Signed Release of Information (ROI) Consent
- Completed and Scored State-Approved Standardized Test
- E508 Assessment Form (submitted within two weeks of the offense(s))
- Clinical Interview
- Assessment Recommendation
During the assessment, The Blanchard Institute will obtain and/or provide the following information:

- Service Agreement (if receiving ongoing care with The Blanchard Institute)
Important things to know about the NC DMV E508 process and our DWI Services:
- Assessments are valid for six months; failure to comply with the NC DMV six-month timeline will require an additional assessment and fee.
- The NC DMV generally will not reinstate the client’s driving privileges unless proof of completion and/or treatment is provided.
- Clients who have been assessed by other facilities can elect to meet the NC DMV educational and/or ongoing care requirements with The Blanchard Institute with proof of their assessment.
